GED Score Report Information
Score Requests
The Testing and Assessment Office issues GED score
reports to individuals that have taken the GED Examination at Jefferson
State Community College. Examinees must submit a written request for each
requested score report. Requests must include the examinee’s full name
(including any maiden and/or previous last names), date of birth, social
security number, the complete mailing address to which the score report
should be mailed, and the examinee’s personal signature. Requests may be
submitted at the ACT Center (Lurleen Wallace Hall, Room 322), or
examinees may mail written requests to:
Jefferson State Community College
Testing and Assessment Office
2601 Carson Road
Birmingham, Alabama 35215
Requests can not be made by phone or email
because the student's signature is required to release his/her score
report.
With the exception of transcripts sent to Jefferson
State Community College Enrollment Services, there is a $5.00 fee for
each requested transcript (payable to Jefferson State Community College).
Score reports issued to examinees will be stamped “Issued to Student.”
Score reports issued to colleges, universities, etc. will bear the
official seal of the college and are considered official. Score report
requests are honored as quickly as possible, in the order of receipt.
Provided that all information on the score report request is accurate and
verifiable, requests are honored within one week of receipt.
